Sign Up for Service

Download and complete English forms: Single Application or Joint Application. Download and complete Spanish forms: Single Application or Joint Application. Bring the form into one of our offices (either in Rapid City or Wall) to complete the service sign up process. You will need a photo ID and must be present in the office to sign up for membership. (if this is a joint membership both parties must be present with ID) If you are unable to come to either office you may download the application and have the bottom portion notarized. You can send or fax the notarized application. We will run a credit check upon receiving your application for service. You may be required to pay a connect fee and/or deposit. If a connect fee, deposit or payment on your prepay is required this must be done within 3 business days or your application will be voided. You can also sign up for service online.
